Alleviating and building back from Jumper's Knee (AKA Patellar Tendinopathy)
Patellar tendinopathy, commonly referred to as "jumper's knee," is a prevalent condition among athletes, particularly those involved in sports that require repetitive jumping or running. Usually patients come to see us when they start to experience pain in the front of the knee, one of the symptoms of this condition.

What is Patellar Tendinopathy?
Patellar tendinopathy is characterized by pain and impaired healing of the patellar tendon, which connects the kneecap (patella) to the shinbone (tibia). This condition arises when the tendon's capacity for load outweighs the healing and recovery. While it can affect anyone, it is most common in athletes who engage in high-impact sports such running or sports that require a lot of jumping.
Causes of Patellar Tendinopathy
There are several factors that contribute to the development of patellar tendinopathy:
Repetitive Stress: Activities involving repetitive jumping, running, or squatting can place excessive strain on the patellar tendon.
Improper Technique: Poor biomechanics during sports or exercises can lead to increased stress on the knee joint.
Muscle Imbalances: Weakness in the quadriceps or tightness in the hamstrings can alter the load distribution on the patellar tendon.
Inadequate Recovery: Not enough sleep, rest, or fueling; or an increase in life stress.
Age and Training Intensity: Older athletes or those who suddenly increase their training intensity are at a higher risk.
Symptoms of Patellar Tendinopathy
Individuals with patellar tendinopathy may experience a variety of symptoms, including:
Knee Pain: Pain is often felt at the front of the knee, just below the kneecap.
Swelling: The area around the patellar tendon may become swollen.
Stiffness: Stiffness in the knee, especially after periods of inactivity.
Weakness: Reduced strength in the knee, particularly during jumping or squatting.
Pain during Activity: Discomfort may worsen during activities that involve jumping or running.
Diagnosis
A healthcare professional will typically diagnose patellar tendinopathy through a physical examination and a review of the patient's medical history.

Treatment Options
Clinch Performance and Recovery is a Reno Physical Therapy clinic where we take a proactive approach to not only get the tendon out of pain, but also get it more durable and stronger so that the athlete can get back to training.
Here are some exercises we have used for some of our athletes to help them resolve their patellar tendinopathy:

Wall sit isometrics: Can be used for pain-alleviation and quadricep muscular endurance.
Heels elevated squats: Used to strengthen the quadricep muscles and patellar tendons.
Quad release with kettlebell: Helps with feelings of tightness and discomfort in the quadricep and knee.
Seated knee extensions: Isolated quadricep and patellar tendon strength.
When undergoing an exercise program, we make sure to keep the intensity at a level that the tendon can tolerate. This is usually a 3 or below on the pain scale (0 being no pain, 10 being the worst it's ever felt). This means we can load the tendon to get it healing while not pushing it over the edge and flaring it up.
Tendons also respond well to higher load, lower repetition exercises. This means the reps are usually kept within the 3 - 6 range, and the load is 80-95% 1 RM.
With this being said, we still want to make sure that the exercises are 100% appropriate for the individual since everyone responds to a program, differently. This is why getting on an individualized physical therapy program is necessary.
Prevention
To reduce the risk of developing patellar tendinopathy, consider the following preventive measures:
Proper Warm-Up: Always warm up before engaging in physical activity to prepare the muscles and tendons.
Strength Training: Incorporate strength training exercises to improve muscle balance and support the knee joint.
Flexibility Exercises: Regularly stretch the quadriceps, hamstrings, and calves to maintain the appropriate amount of flexibility that is required of the activity.
Gradual Progression: Increase training intensity gradually to avoid overloading the tendon.
